How to Request an Accommodation
1. Complete the Form
Download and complete the official Special Accommodations Request Form.
2. Gather Documentation
Obtain recent, comprehensive documentation from a qualified healthcare professional.
3. Submit with Application
Upload the completed form and all supporting documents with your online exam application.
Documentation Requirements
To ensure a thorough and timely review, your request must be accompanied by documentation from a qualified healthcare professional that includes:
- The specific diagnosis and nature of the disability.
- A description of the functional limitations that impact your ability to take the exam under standard conditions.
- The specific accommodation(s) requested and a rationale for why they are necessary.
- Documentation must be recent and relevant to your current condition.
Review Process & Confidentiality
Your privacy is important to us. All accommodation requests and supporting documents are kept confidential and are reviewed separately from your certification application.
- Requests are reviewed on a case-by-case basis by trained staff.
- Please allow an additional 15-20 business days for the review of your accommodation request.
- You will be notified of the decision via email and your online candidate portal.
