An Anticoagulation Clinic Pharmacist is a specialized ambulatory care provider who manages and monitors patients on warfarin (Coumadin) and Direct Oral Anticoagulants (DOACs). Working under a collaborative practice agreement, these pharmacists are the primary point of contact for patients taking these high-risk medications for conditions like atrial fibrillation, deep vein thrombosis (DVT), and pulmonary embolism (PE).
This role is defined by clinical autonomy and direct patient management. You will perform point-of-care INR testing, make real-time warfarin dose adjustments, manage peri-procedural bridging plans, and educate patients on drug interactions and dietary considerations. Your expertise is critical in balancing the fine line between preventing blood clots and minimizing bleeding risk, making you an indispensable part of the patient care team.
Core Responsibilities

INR Monitoring & Warfarin Dosing
Conducting fingerstick INR tests, assessing results, and making immediate, independent dose adjustments based on established protocols.

DOAC Management & Counseling
Ensuring appropriate patient selection, dosing for renal function, and providing crucial counseling on adherence and bleeding risks for medications like Eliquis and Xarelto.

Peri-procedural Bridging
Developing safe and effective plans to stop and restart anticoagulation therapy for patients undergoing surgeries or invasive procedures.

Drug Interaction Management
Proactively identifying and managing potential interactions with new medications, including antibiotics and OTC products, that can affect anticoagulation status.
