A Cardiology Clinic Ambulatory Care Pharmacist is a clinical specialist who works directly with patients and cardiologists in an outpatient setting. This role moves beyond traditional dispensing to focus on comprehensive medication management for chronic cardiovascular conditions like heart failure, hypertension, hyperlipidemia, and atrial fibrillation. You are the team’s expert on cardiovascular pharmacotherapy.

Operating under a collaborative practice agreement, you will manage patient panels, initiate and adjust medications, order lab tests, and provide intensive patient education. Your goal is to optimize evidence-based therapies, improve medication adherence, and prevent hospital readmissions. This is a hands-on, patient-facing role that requires sharp clinical skills and the ability to build strong, trusting relationships with both patients and providers.

Core Responsibilities

  • Chronic Disease State Management

    Titrating guideline-directed medical therapy for heart failure, managing complex antiarrhythmic and anticoagulant regimens, and achieving blood pressure and lipid goals.

  • Collaborative Patient Visits

    Conducting in-person, telephonic, or virtual visits to assess medication adherence, screen for side effects, and provide patient education.

  • Device and Data Monitoring

    Reviewing data from remote patient monitoring devices like blood pressure cuffs or weight scales to make proactive medication adjustments.

  • Transitions of Care

    Performing post-discharge medication reconciliation for patients recently hospitalized for a cardiac event to prevent errors and readmissions.
