A Clinical Pharmacist Practitioner (CPP) represents one of the highest levels of clinical autonomy in the pharmacy profession. Operating under a formal Collaborative Practice Agreement (CPA) with physicians, CPPs function as licensed, independent providers with prescriptive authority. They are authorized to perform patient assessments, order and interpret laboratory tests, and initiate, adjust, or discontinue medication therapies.

As a CPP, you are an integral member of the patient’s care team, often managing your own panel of patients with chronic diseases like diabetes, hypertension, or heart failure. You work in ambulatory care clinics, physicians’ offices, and health systems, providing comprehensive medication management and direct patient care that goes far beyond traditional dispensing, directly improving health outcomes and reducing the burden on physicians.
