Follow this step-by-step guide to navigate the application process and begin your journey toward CPS certification.

Create Your Account

Your journey begins at the secure CPS online portal. You will need to create a personal account to start your application. You can save your progress and return at any time before submission.

Complete the Online Application

The step-by-step online form will guide you through providing detailed information about your education, licensure, and professional experience. It is your responsibility to ensure all information is accurate and complete, as incomplete applications will not be reviewed.

Upload Required Documents

You must upload clear, legible PDF copies of the following documents to the portal:

  • Proof of Education: Official transcript or diploma showing your pharmacy degree and graduation date.
  • Proof of Licensure: A copy of your active pharmacy license showing your name, license number, status, and expiration date.
  • Curriculum Vitae (CV) or Resume: A current copy detailing your professional experience.
  • Experience Verification Form: If applying via the Standard Experience pathway.
Special Accommodations: If you require testing accommodations under the ADA, you must submit the official request form and supporting documentation along with your application.

Submit Fees & Payment

All fees are required at the time of submission to complete your application. Payments are processed securely online via credit card (Visa, MasterCard, American Express).

  • Application Fee: $395 (Non-refundable)
Applications are not considered complete until payment has been successfully processed. CPS does not accept personal checks or purchase orders.

Application Review

Once submitted, our staff will review your complete application, documentation, and payment. The review process typically takes 10-15 business days. You will be notified of the outcome via email and your online CPS portal.

  • Approved: You will receive an Authorization to Test (ATT) and can schedule your exam.
  • Incomplete: You will be notified of any missing information and will have 30 days to provide it.
  • Denied: You will receive an explanation if your application does not meet the eligibility requirements.
