Their primary goal is to ensure that every patient’s medication regimen is safe, effective, and evidence-based.

This role is highly dynamic and involves real-time clinical decision-making. Responsibilities include performing admission and discharge medication reconciliation, providing pharmacokinetic monitoring for drugs with narrow therapeutic windows, answering complex drug information questions, and intervening to prevent medication errors. They are the go-to experts for everything medication-related, from dose adjustments in renal impairment to selecting the optimal antibiotic, making a direct impact on patient outcomes every day.

    Conducting best-possible medication histories on admission and ensuring seamless medication continuity upon discharge to prevent errors.

    Participating in patient care rounds to provide real-time pharmacotherapy recommendations and serve as the team’s drug expert.

  • Transitions of Care

    Ensuring patients understand their discharge medications through counseling and coordination with outpatient pharmacies and providers.
